Who this covers

This policy covers the website at dsmoc.com. It does not cover your medical record, which is protected health information handled under HIPAA and described in the practice's Notice of Privacy Practices, provided to you as a patient.
What this site collects
Very little, by design. There are no contact forms, no appointment booking, no accounts, and no newsletter signup on this website. Nothing on it asks you to type anything.
- Information you choose to send. If you use a phone or email link on this site, we receive whatever you decide to tell us in that call or message.
- Server logs. Our hosting provider records standard technical information for security and reliability, such as IP address, browser type, referring page, and the pages requested. We do not use these logs to build a profile of you.
- Local storage. The site sets no first-party cookies and stores nothing in your browser.
Analytics and advertising
As published today, this site runs no analytics, no advertising pixels, no conversion tracking, and no session recording. If analytics are added in future, this section will be updated to name the provider and describe what is collected, and a consent mechanism will be added if one is required.
Third-party content
- Google Maps. The contact page embeds a Google map. Loading it sends your request to Google, which may set cookies and collect data under Google's privacy policy. The same address and phone number are also on the page as plain text, so you never have to load the map to get the information.
- Fonts. Typefaces are self-hosted on this site's own domain. No request is made to Google Fonts or any other font service.
- Video. Videos are hosted on this site's own domain and play only after you click. No video is streamed from YouTube, Vimeo, or any third party, and no video request is made until you ask for one.
Please do not send health details through the website
Email is not a secure channel for protected health information. Do not include medical details, insurance identifiers, or other sensitive information in an email. Call the office and we will collect what is needed securely.
How information is used
Information you send is used only to respond to you and to coordinate your care. Technical logs are used only to operate and secure the site. We do not sell personal information and we do not share it for cross-context behavioral advertising.
Children
This site is not directed to children under 13 and does not knowingly collect their information.
Your choices
You can contact us at any time to ask what we hold about you and to request correction or deletion, subject to our recordkeeping and legal obligations. Depending on where you live, additional privacy rights may apply. Your browser's Do Not Track or Global Privacy Control signal has nothing to act on here, because the site performs no tracking.
Changes
If the site's behavior changes, for example if analytics or a contact form is added, this policy will be updated and the date at the top will change with it.
